When tests have established the presence of a kidney stone, the next step is to determine treatment. The patient should be admitted to the emergency room if vomiting is severe, if fever is present, or if symptoms indicate an infection. X-rays are usually warranted at that time to determine the presence of a stone. Strong opioid painkillers such as meperidine Demerol ; are often required for a severe kidney stone attack, although physicians will usually not administer them until the presence of a kidney stone has been confirmed on the x-ray. In some cases, administration of powerful nonsteroidal anti-inflammatory drugs NSAIDs ; may be as effective as opioids and they have fewer side effects. They do take longer to have an effect, however.

The risk of MI increased 11-fold in women with a prothrombin mutation 20210GA who took HRT and had hypertension.34 As a result of the WHI findings, new language has been inserted into the CEE, 11 combination CEE with MPA CEE MPA ; , 35 and conjugated estrogens A, synthetic, 36 prescribing information. The new language essentially states that the WHI study reported increased risks of MI, stroke, invasive breast cancer, pulmonary edema, and DVT in postmenopausal women during 5 years of treatment with CEE 0.625 mg combined with MPA 2.5 mg relative to placebo. Other doses of conjugated estrogens and MPA and other combinations of estrogens and progestins were not studied in the WHI and, in the absence of comparable data, these risks should be assumed to be similar. Therefore estrogens with or without progestins should be prescribed at the lowest effective doses and for the shortest duration.11, 35, 36 In other words, these are class effects and not product specific. Note: this point differs from the actual monograph presentation to the P&T committee in February 2003 and is updated for this article. ; The FDA announced on January 8, 2003, that all labels on estrogen and estrogen-progestin replacement therapy will be revised to carry a boxed warning that identifies the increased risks of heart disease, heart attacks, strokes, and breast cancer and that these products are not approved for heart disease prevention. Estrogen is still recommended for hot flushes and night sweats but should be used at the lowest doses and for the shortest possible time. For vaginal and vulvar atrophy, vaginal estrogen products are suggested. New labels will state that before using estrogen to prevent osteoporosis, alternative nonestrogen therapies should be considered.37 In the June 2003 prescribing information for the CEE MPA combination product, the labeling states, "When prescribing solely for the treatment of symptoms of vulvar and vaginal atrophy, topical vaginal products should be considered When prescribing solely for the prevention of postmenopausal osteoporosis, therapy should only be considered for women at significant risk of osteoporosis and nonestrogen medications should be carefully considered."35 Note: this point differs from the actual monograph presentation to the P&T committee in February 2003 and is updated for this article. ; Adverse Events. We are purposefully trying to reduce the Focus committee size and make greater use of our professional organisers. The theme for the meeting will be "Focus beyond the laboratory" and will look at areas such as: communication, changing user base, service provision models, governance and patient public involvement. At Focus 2007 we are trying to look at different options for participation by industry. We are open minded about the type and style of commercial input with 2000 m2 of space available for use in whatever way is decided. Over the next few weeks we will be talking with Corporate Members regarding this. There was some discussion about problems that Corporate Members had experienced this year with getting details of the Brighton meeting. Our new meeting organisers have been on a steep learning curve and certainly it was acknowledged that information to Corporate Members is an area that is now receiving close attention. Progress with the UK version of Lab Tests Online was reviewed by Mike Hallworth. The site is now getting 70, 000 visitors a month with a dwell time averaging 7-12 minutes. Other healthcare professionals are also using the site. New tests are being added and Peter Hooper's suggestion of adding osmolality has been implemented! The Department of Health is funding the site for the next three years. We do not need additonal funding for the site but Mike encouraged Corporate Members to consider helping with publicity about the site in company newsletters and any other way they could. The following question has been extracted from a letter to the editor, regarding the article "notes on assessing lifestyle and cardiac risk" in issue 8 of the breakspear medical bulletin.

Membrane inlet mass spectrometry MIMS ; is an established technique for the analysis of volatile organic compounds in aqueous solutions and in air. A thin membrane is the only interface between a liquid or gaseous sample at atmospheric pressure and the vacuum of a mass spectrometer. Since its introduction about 35 years ago MIMS has been applied mainly in biochemistry and environmental analysis. In this work the applicability of MIMS in the analysis of volatile organic compounds VOCs ; in water samples was investigated, and improved MIMS methods for the analysis of VOCs in air samples were constructed. The possibilities of MIMS for the analysis of polar and or semivolatile compounds in aqueous samples were enhanced with novel techniques. It was demonstrated that the MIMS method is comparable with static headspace gas chromatography and purge&trap gas chromatography-mass spectrometry methods in the analysis of VOCs in water samples. The MIMS method was also shown to be very suitable for on-site measurement of water samples in a mobile laboratory. A membrane inlet mass spectrometric method was developed for the analysis of volatile organic compounds, especially volatile sulfur compounds, in air samples. The method is very sensitive, i.e. detection limits are at sub or low g m3 levels, and also very rapid: it is possible to analyze even 50 to 100 samples in one hour with a thin polydimethylsiloxane membrane because response times are only a few seconds. When MIMS is combined with a temperature-programmed desorption TPD ; technique it is possible to achieve.

Famotidine uses: used to treat and prevent the recurrence of ulcers and to treat other conditions where the stomach makes too much acid.

It does look a little bit different. I think we are going to find that there are a whole bunch of different subtypes- what we decide to call them is a good question. I think that's where the biological studies come in. Let's look at some genetics; let's look at some biological markers. Put the two together and this is what it means to have this particular type of bipolar disorder. I think terminology is not going to be as important as understanding the different subtypes and what treatments work for each. CABF: Do you think there will come a point where neuroimaging will be used diagnostically for bipolar? KC: Absolutely. It may not be that we'll say, "Yes, you have bipolar disorder" by looking at the scan. It may be, "Here's what you have and here's how to treat it". Again, avoiding the whole terminology question. That kind of diagnostic process would be great and I think that's the right direction to go, but we're not there yet. CABF: How far do you think we are from that? KC: I used to think we were a long way off, but now I'm thinking maybe five to ten years. Things are moving so much quicker. Just in the last few years we've come so far. If we look at the last ten years and the amount of treatment that's come out, how much brain based research is being done, it's remarkable. We're making a lot of headway. CABF: Is the neuroimaging study contributing any genetic information? KC: We are going to combine the two. We just started the genetic piece with our families from the offspring study. We're going to collect blood samples to isolate DNA from all of these study families. This study is going to be unique because we are going to able to look at children who have bipolar disorder, parents who have it, and the children who maybe have early forms. We already know it's familial in these families, so if there are going to be at-risk genes anywhere, we should see them in these families. This is a very specific, homogenous group we're looking at. Hopefully, our findings will apply to all kinds of bipolar disorders, but we don't know. Maybe it will only apply to familial bipolar disorder, but we're hoping it will apply to all kinds of bipolar disorder. CABF: What is your hypothesis for the genetic study? KC: There are a few hypotheses. The part that I'm really excited about is investigating the validity of genetic anticipation in bipolar disorder, the idea that with every generation the disorder is showing up at an earlier age and with increased severity. We are hoping to do a screen of their genomes and see if there are trinucleotide repeats in certain areas, and if so if the genes they are near could actually be related to the bipolar disorder. We'll also be looking for other patterns. It's daunting research because there is so much potential data to look at, but we'll have cell lines so we can always go back and look at the DNA again for other possible clues. CABF: Who is funding your research? KC: Other than the NIMH grant, we have a grant from the Heinz Prechter Fund for the genetic study. They have given us a very generous grant to study this for two years. The families in our studies have been wonderful and they understand that this may not give us clinical data that will help their child, but they are contributing to important research for the future. NARSAD and the Klingenstein Third Generation Foundation has also been fantastic in getting our neuroimaging studies off the ground. We also receive industry funding for some of our medication efficacy research. CABF: Let's talk about some clinical issues. Does the Stanford clinic currently use the BP-NOS diagnosis for bipolar children? KC: We do in the clinic, absolutely, but we try to standardize it. We look for the symptoms to be there predominantly- not every minute of the day, but predominantly there for a distinct period. And it helps to have the child's family history. If a child has a number of mania symptoms and a loaded genetic history for mood disorders you can then say it's probably in the bipolar spectrum- and then it's a question of whether it's an early or subsyndromal presentation. Background: Most women worry to some extent during pregnancy about exposure to agents that might harm their babies. Cases: We describe three women who worried excessively throughout pregnancy about harming their babies because of exposure to agents including, but not limited to, psychotropic drugs. These women were extremely resistant to reassurances that their babies would not be adversely affected, and it is likely there are more women in the community who fit this profile. We have described a number of management strategies that we found effective in caring for these women during pregnancy. Conclusion: A collaborative effort between caregivers in psychiatry and obstetrics, as well as other health professionals, is required to provide management for these women during pregnancy. A rare, but potentially fatal adverse effect of aspirin called Reye's syndrome, that occurs exclusively in children, is less known. Reye's syndrome is an acute illness, characterised by acute noninflammatory encephalopathy and deterioration of liver function. The same symptoms may occur in some inborn metabolic disorders and Reye's syndrome can only be distinguished from these hereditary disorders by electron microscopic examination of liver biopsy tissue. Acute encephalopathy without reasonable explanation for cerebral abnormalities together with liver dysfunction manifested by elevated ammonia or liver-enzyme concentrations became the clinical definition of Reye's syndrome. Depending on the severity of the symptoms the outcome of Reye's syndrome can range from complete recovery through mild to severe permanent neurological damage to death. Epidemiological studies showed that about 30% of children who presents with Reye's syndrome die or suffer from longterm neurological complications. Mortality is significantly increased in children of less than five years of age, by presence of diarrhoea during antecedent viral illness, by black race, by serum ammonia levels above 26 umol L and serum glucose levels below 3.3 mmol L. The most widely available non-steroidal analgesic and antiinflammatory agent in Zimbabwe is aspirin. It is available in pharmacies and food stores, supermarkets, etc. the other alternative minor analgesic-antipyretic agent, paracetamol products are restricted to sale within pharmacies. There is no data available on incidence of Reye's syndrome in Zimbabwe. However, considering the strong evidence from international studies, Reye's syndrome undoubtly occurs in children in Zimbabwe, even though it is not diagnosed as such or associated with aspirin use.
